Step 1

Step 1: Submit Requirements

Fill out the membership form. The first documents required upon application are a Photocopy of a Valid ID and 2 pcs 2×2 ID Pictures. Other documents may be submitted afterward as part of the application process.

    • Barangay Clearance

    • Photocopy of Valid ID

    • 2pcs 2×2 ID Pictures

    • Proof of Income

Step 2

Step 2: Attend Seminar

Attend the Pre-Membership Education Seminar (PMES), an essential part of the application process designed to help you fully understand the cooperative. During this session, you will be introduced to the different services and programs available to members.
The seminar also explains your rights, responsibilities, and the benefits you can expect as part of the cooperative. In addition, applicants will undergo membership screening and orientation to assess eligibility and ensure that each individual is prepared to actively participate and contribute to the growth of the cooperative.



Step 3

Step 3: Wait for Approval

After completing all the required documents and successfully attending the seminar, your application will proceed to the evaluation and approval stage. During this process, the cooperative carefully reviews your submitted information to ensure compliance with membership requirements and policies.
Approval of applications is typically conducted during scheduled processing days, which are held on the 2nd or 4th Saturday of each month at the OLLSMPC main office. Once your application is approved, you will officially become a registered member of the cooperative, granting you access to its full range of services, financial programs, and member-exclusive benefits designed to support your growth and stability.
